There are numerous mathematically equivalent formulations of quantum mechanics. One of the oldest and most commonly used formulations is the transformation theory proposed by Cambridge theoretical physicist Paul Dirac, which unifies and generalizes the two earliest formulations of quantum mechanics, matrix mechanics (invented by Werner Heisenberg)[2] and wave mechanics (invented by Erwin Schrödinger).

In this formulation, the instantaneous state of a quantum system encodes the probabilities of its measurable properties, or "observables". Examples of observables include energy, position, momentum, and angular momentum. Observables can be either continuous (e.g., the position of a particle) or discrete (e.g., the energy of an electron bound to a hydrogen atom).

Generally, quantum mechanics does not assign definite values to observables. Instead, it makes predictions about probability distributions; that is, the probability of obtaining each of the possible outcomes from measuring an observable. Naturally, these probabilities will depend on the quantum state at the instant of the measurement. There are, however, certain states that are associated with a definite value of a particular observable. These are known as "eigenstates" of the observable ("eigen" can be roughly translated from German as inherent or as a characteristic). In the everyday world, it is natural and intuitive to think of everything being in an eigenstate of every observable. Everything appears to have a definite position, a definite momentum, and a definite time of occurrence. However, quantum mechanics does not pinpoint the exact values for the position or momentum of a certain particle in a given space in a finite time; rather, it only provides a range of probabilities of where that particle might be. Therefore, it became necessary to use different words for (a) the state of something having an uncertainty relation and (b) a state that has a definite value. The latter is called the "eigenstate" of the property being measured.

For example, consider a free particle. In quantum mechanics, there is wave-particle duality so the properties of the particle can be described as a wave. Therefore, its quantum state can be represented as a wave, of arbitrary shape and extending over all of space, called a wave function. The position and momentum of the particle are observables. The Uncertainty Principle of quantum mechanics states that both the position and the momentum cannot simultaneously be known with infinite precision at the same time. However, one can measure just the position alone of a moving free particle creating an eigenstate of position with a wavefunction that is very large at a particular position x, and almost zero everywhere else. If one performs a position measurement on such a wavefunction, the result x will be obtained with almost 100% probability. In other words, the position of the free particle will almost be known. This is called an eigenstate of position (mathematically more precise: a generalized eigenstate (eigendistribution) ). If the particle is in an eigenstate of position then its momentum is completely unknown. An eigenstate of momentum, on the other hand, has the form of a plane wave. It can be shown that the wavelength is equal to h/p, where h is Planck's constant and p is the momentum of the eigenstate. If the particle is in an eigenstate of momentum then its position is completely blurred out.

Usually, a system will not be in an eigenstate of whatever observable we are interested in. However, if one measures the observable, the wavefunction will instantaneously be an eigenstate (or generalized eigenstate) of that observable. This process is known as wavefunction collapse. It involves expanding the system under study to include the measurement device, so that a detailed quantum calculation would no longer be feasible and a classical description must be used. If one knows the corresponding wave function at the instant before the measurement, one will be able to compute the probability of collapsing into each of the possible eigenstates. For example, the free particle in the previous example will usually have a wavefunction that is a wave packet centered around some mean position x0, neither an eigenstate of position nor of momentum. When one measures the position of the particle, it is impossible to predict with certainty the result that we will obtain. It is probable, but not certain, that it will be near x0, where the amplitude of the wave function is large. After the measurement is performed, having obtained some result x, the wave function collapses into a position eigenstate centered at x.

Wave functions can change as time progresses. An equation known as the Schrödinger equation describes how wave functions change in time, a role similar to Newton's second law in classical mechanics. The Schrödinger equation, applied to the aforementioned example of the free particle, predicts that the center of a wave packet will move through space at a constant velocity, like a classical particle with no forces acting on it. However, the wave packet will also spread out as time progresses, which means that the position becomes more uncertain. This also has the effect of turning position eigenstates (which can be thought of as infinitely sharp wave packets) into broadened wave packets that are no longer position eigenstates.

Some wave functions produce probability distributions that are constant in time. Many systems that are treated dynamically in classical mechanics are described by such "static" wave functions. For example, a single electron in an unexcited atom is pictured classically as a particle moving in a circular trajectory around the atomic nucleus, whereas in quantum mechanics it is described by a static, spherically symmetric wavefunction surrounding the nucleus (Fig. 1). (Note that only the lowest angular momentum states, labeled s, are spherically symmetric).

The time evolution of wave functions is deterministic in the sense that, given a wavefunction at an initial time, it makes a definite prediction of what the wavefunction will be at any later time. During a measurement, the change of the wavefunction into another one is not deterministic, but rather unpredictable, i.e., random.

The probabilistic nature of quantum mechanics thus stems from the act of measurement. This is one of the most difficult aspects of quantum systems to understand. It was the central topic in the famous Bohr-Einstein debates, in which the two scientists attempted to clarify these fundamental principles by way of thought experiments. In the decades after the formulation of quantum mechanics, the question of what constitutes a "measurement" has been extensively studied. Interpretations of quantum mechanics have been formulated to do away with the concept of "wavefunction collapse"; see, for example, the relative state interpretation. The basic idea is that when a quantum system interacts with a measuring apparatus, their respective wavefunctions become entangled, so that the original quantum system ceases to exist as an independent entity.

Chaotic behavior has been observed in the laboratory in a variety of systems including electrical circuits, lasers, oscillating chemical reactions, fluid dynamics, and mechanical and magneto-mechanical devices. Observations of chaotic behaviour in nature include the dynamics of satellites in the solar system, the time evolution of the magnetic field of celestial bodies, population growth in ecology, the dynamics of the action potentials in neurons, and molecular vibrations. Everyday examples of chaotic systems include weather and climate.[1] There is some controversy over the existence of chaotic dynamics in the plate tectonics and in economics.[2][3][4]

Systems that exhibit mathematical chaos are deterministic and thus orderly in some sense; this technical use of the word chaos is at odds with common parlance, which suggests complete disorder. A related field of physics called quantum chaos theory studies systems that follow the laws of quantum mechanics. Recently, another field, called relativistic chaos,[5] has emerged to describe systems that follow the laws of general relativity.

As well as being orderly in the sense of being deterministic, chaotic systems usually have well defined statistics.[citation needed] For example, the Lorenz system pictured is chaotic, but has a clearly defined structure. Bounded chaos is a useful term for describing models of disorder.

The first discoverer of chaos was Henri Poincaré. In 1890, while studying the three-body problem, he found that there can be orbits which are nonperiodic, and yet not forever increasing nor approaching a fixed point.[6] In 1898 Jacques Hadamard published an influential study of the chaotic motion of a free particle gliding frictionlessly on a surface of constant negative curvature.[7] In the system studied, "Hadamard's billiards," Hadamard was able to show that all trajectories are unstable in that all particle trajectories diverge exponentially from one another, with a positive Lyapunov exponent.

Much of the earlier theory was developed almost entirely by mathematicians, under the name of ergodic theory. Later studies, also on the topic of nonlinear differential equations, were carried out by G.D. Birkhoff[8], A.N. Kolmogorov[9][10][11], M.L. Cartwright and J.E. Littlewood[12], and Stephen Smale.[13] Except for Smale, these studies were all directly inspired by physics: the three-body problem in the case of Birkhoff, turbulence and astronomical problems in the case of Kolmogorov, and radio engineering in the case of Cartwright and Littlewood. Although chaotic planetary motion had not been observed, experimentalists had encountered turbulence in fluid motion and nonperiodic oscillation in radio circuits without the benefit of a theory to explain what they were seeing.

Despite initial insights in the first half of the twentieth century, chaos theory became formalized as such only after mid-century, when it first became evident for some scientists that linear theory, the prevailing system theory at that time, simply could not explain the observed behaviour of certain experiments like that of the logistic map. What had been beforehand excluded as measure imprecision and simple "noise" was considered by chaos theories as a full component of the studied systems.

The main catalyst for the development of chaos theory was the electronic computer. Much of the mathematics of chaos theory involves the repeated iteration of simple mathematical formulas, which would be impractical to do by hand. Electronic computers made these repeated calculations practical, while figures and images made it possible to visualize these systems. One of the earliest electronic digital computers, ENIAC, was used to run simple weather forecasting models.

Studies of the critical point beyond which a system creates turbulence was important for Chaos theory, analyzed for example by the Soviet physicist Lev Landau who developed the Landau-Hopf theory of turbulence. David Ruelle and Floris Takens later predicted, against Landau, that fluid turbulence could develop through a strange attractor, a main concept of chaos theory.

An early pioneer of the theory was Edward Lorenz whose interest in chaos came about accidentally through his work on weather prediction in 1961.[14] Lorenz was using a simple digital computer, a Royal McBee LGP-30, to run his weather simulation. He wanted to see a sequence of data again and to save time he started the simulation in the middle of its course. He was able to do this by entering a printout of the data corresponding to conditions in the middle of his simulation which he had calculated last time.

To his surprise the weather that the machine began to predict was completely different from the weather calculated before. Lorenz tracked this down to the computer printout. The computer worked with 6-digit precision, but the printout rounded variables off to a 3-digit number, so a value like 0.506127 was printed as 0.506. This difference is tiny and the consensus at the time would have been that it should have had practically no effect. However Lorenz had discovered that small changes in initial conditions produced large changes in the long-term outcome.[15] Lorenz's discovery, which gave its name to Lorenz attractors, proved that meteorology could not reasonably predict weather beyond a weekly period (at most).

The year before, Benoit Mandelbrot found recurring patterns at every scale in data on cotton prices.[16] Beforehand, he had studied information theory and concluded noise was patterned like a Cantor set: on any scale the proportion of noise-containing periods to error-free periods was a constant-- thus errors were inevitable and must be planned for by incorporating redundancy.[17] Mandelbrot described both the "Noah effect" (in which sudden discontinuous changes can occur, e.g., in a stock's prices after bad news, thus challenging normal distribution theory in statistics, aka Bell Curve) and the "Joseph effect" (in which persistence of a value can occur for a while, yet suddenly change afterwards).[18][19] In 1967, he published "How long is the coast of Britain? Statistical self-similarity and fractional dimension," showing that a coastline's length varies with the scale of the measuring instrument, resembles itself at all scales, and is infinite in length for an infinitesimally small measuring device.[20] Arguing that a ball of twine appears to be a point when viewed from far away (0-dimensional), a ball when viewed from fairly near (3-dimensional), or a curved strand (1-dimensional), he argued that the dimensions of an object are relative to the observer and may be fractional. An object whose irregularity is constant over different scales ("self-similarity") is a fractal (for example, the Koch curve or "snowflake", which is infinitely long yet encloses a finite space and has fractal dimension equal to circa 1.2619, the Menger sponge and the Sierpiński gasket). In 1975 Mandelbrot published The Fractal Geometry of Nature, which became a classic of chaos theory. Biological systems such as the branching of the circulatory and bronchial systems proved to fit a fractal model.

Chaos was observed by a number of experimenters before it was recognized; e.g., in 1927 by van der Pol[21] and in 1958 by R.L. Ives.[22][23] However, Yoshisuke Ueda seems to have been the first experimenter to have identified a chaotic phenomenon as such by using an analog computer on November 27, 1961. The chaos exhibited by an analog computer is a real phenomenon, in contrast with those that digital computers calculate, which has a different kind of limit on precision. Ueda's supervising professor, Hayashi, did not believe in chaos, and thus he prohibited Ueda from publishing his findings until 1970.[24]

In December 1977 the New York Academy of Sciences organized the first symposium on Chaos, attended by David Ruelle, Robert May, James Yorke (coiner of the term "chaos" as used in mathematics), Robert Shaw (a physicist, part of the Eudaemons group with J. Doyne Farmer and Norman Packard who tried to find a mathematical method to beat roulette, and then created with them the Dynamical Systems Collective in Santa Cruz), and the meteorologist Edward Lorenz.

The following year, Mitchell Feigenbaum published the noted article "Quantitative Universality for a Class of Nonlinear Transformations", where he described logistic maps.[25] Feigenbaum had applied fractal geometry to the study of natural forms such as coastlines. Feigenbaum notably discovered the universality in chaos, permitting an application of chaos theory to many different phenomena.

In 1979, Albert J. Libchaber, during a symposium organized in Aspen by Pierre Hohenberg, presented his experimental observation of the bifurcation cascade that leads to chaos and turbulence in convective Rayleigh-Benard systems. He was awarded the Wolf Prize in Physics in 1986 along with Mitchell J. Feigenbaum "for his brilliant experimental demonstration of the transition to turbulence and chaos in dynamical systems".[26]

The New York Academy of Sciences then co-organized, in 1986, with the National Institute of Mental Health and the Office of Naval Research the first important conference on Chaos in biology and medicine. Bernardo Huberman thereby presented a mathematical model of the eye tracking disorder among schizophrenics [27]. Chaos theory thereafter renewed physiology in the 1980s, for example in the study of pathological cardiac cycles.

Normal variations

Paraphimosis is an inability to move the foreskin forward, over the glans. It can result from fluid trapped in a foreskin which is left retracted, perhaps following a medical procedure, or accumulation of fluid in the foreskin because of friction during vigorous sexual activity.

In Peyronie's disease, anomalous scar tissue grows in the soft tissue of the penis, causing curvature. Severe cases can benefit from surgical correction.

A thrombosis can occur during periods of frequent and prolonged sexual activity, especially fellatio. It is usually harmless and self-corrects within a few weeks.

Infection with the Herpes virus can occur after sexual contact with an infected carrier; this may lead to the development of herpes sores.

Pudendal nerve entrapment is a condition characterized by pain on sitting and loss of penile (or clitoral) sensation and orgasm. Occasionally there is a total loss of sensation and orgasm. The pudendal nerve can be damaged by narrow hard bicycle seats and accidents.

Penile fracture can occur if the erect penis is bent excessively. A popping or cracking sound and pain is normally associated with this event. Emergency medical assistance should be obtained. Prompt medical attention lowers likelihood of permanent penile curvature.

In diabetes, peripheral neuropathy can cause tingling in the penile skin and possibly reduced or completely absent sensation. The reduced sensations can lead to injuries for either partner and their absence can make it impossible to have sexual pleasure through stimulation of the penis. Since the problems are caused by permanent nerve damage, preventive treatment through good control of the diabetes is the primary treatment. Some limited recovery may be possible through improved diabetes control.

Erectile dysfunction or impotence is the inability to have and maintain an erection sufficiently firm for satisfactory sexual performance. Diabetes is a leading cause, as is natural aging. A variety of treatments exist, including drugs, such as sildenafil citrate (marketed as Viagra) which works by vasodilation.

Priapism is a painful and potentially harmful medical condition in which the erect penis does not return to its flaccid state. The causative mechanisms are poorly understood but involve complex neurological and vascular factors. Potential complications include ischaemia, thrombosis, and impotence. In serious cases the condition may result in gangrene, which may necessitate amputation. The condition has been associated with a variety of drugs including prostaglandin but not Sildenafil (Viagra)[4].

Lymphangiosclerosis is a hardened lymph vessel, although it can feel like a hardened, almost calcified or fibrous, vein. It tends not to share the common blue tint with a vein however. It can be felt as a hardened lump or "vein" even when the penis is flaccid, and is even more prominent during an erection. It is considered a benign physical condition. It is fairly common and can follow a particularly vigorous sexual activity for men and tend to go away if given rest and more gentle care, for example by use of lubricants.

Carcinoma of the penis is rare with a reported rate of 1 person in 100,000 in developed countries. Circumcision is said to protect against this disease but this notion remains controversial[5].

Developmental disorders

Hypospadias is a developmental disorder where the meatus is positioned wrongly at birth. Hypospadias can also occur iatrogenically by the downward pressure of an indwelling urethral catheter.[6] It is usually corrected by surgery. The Intersex Society of North America classifies hypospadias as an intersex condition. They believe in halting all medically unnecessary surgeries, including many of those done on people with hypospadias.

A micropenis is a very small penis caused by developmental or congenital problems. Diphallia, or penile duplication (PD), is the condition of having two penises. However, this disorder is exceedingly rare.

Alleged and observed psychological disorders

* penis panic (koro in Malaysian/Indonesian) - delusion of shrinkage of the penis and retraction into the body. This appears to be culturally conditioned and largely limited to Ghana, Sudan, China, Japan, Southeast Asia, and West Africa
* In April, 2008, Kinshasa, Democratic Republic of Congo, West Africa's 'Police arrested 14 suspected victims (of penis snatching) and sorcerers accused of using black magic or witchcraft to steal (make disappear) or shrink men's penises to extort cash for cure, amid a wave of panic. Arrests were made in an effort to avoid bloodshed seen in Ghana a decade ago, when 12 penis snatchers were beaten to death by mobs.[7]
* penis envy - the contested Freudian belief of a woman envying men for having a penis.
* small penis syndrome - disorder when men believe that their penis is smaller than average
